За разлика от мигрената, кластърното главоболие среща по-рядко, 4-6 пъти по-често при мъже и при черната раса. Етиологията не е известна. Около 70% от пациентите са пушачи и се предполага, че хроничното дразнене на ганглия от цигарен дим може да предизвика едностранна хиперреактивност. Предполага се, че хипокретин 2 рецепторния ген (HERTR2) модулира риска от поява на заболяването. Патофизиологията е свързана с активирането на тригеминоваскуларната система. Циркадната ритмичност на главоболието е свързана с участието на супрахиазмалното ядро и адаптационни промени в хипоталамичните функции. Клинично началото на заболяването е по-късно, отколкото при мигрената, около 28-годишна възраст. Протича с пристъпи от главоболие, придружени от автономни симптоми. Продромалните симптоми предшестват пристъпа от главоболие. Включват летаргия, промени в настроението (депресия, апатия, тревожност, еуфория или хипоманиакално поведение), последвано след няколко минути от неврологични симптоми, като дезориентираност, сънливост и прозяване, раздразнителност, парестезии и странни усещания по тялото. Пристъпът от главоболие започва внезапно и бързо с умерена, винаги едностранна болка в и около окото (ретроорбитално) или в темпоралната област, последвана от запушване на носа. Пристъпът продължава между 15 минути и 3 часа, най-често – от 30 минути до 2 часа. Болката винаги е непоносимо силна, като в началото и края на пристъпа е по-слаба. През денонощието пристъпите имат тенденция да се получават по едно и също време, характерно за пациента, като часовник. Най-често това става между 1 и 2 часа през нощта, а през деня между 13 и 15 часа или около 21 часа. честотата на пристъпите е много индивидуална и варира от един през ден до 8 (средно 4) дневно.
Локалните краниални автономни симптоми се развиват на ипсилатералната на главоболието страна. Включват лакримация (при 90%), инекция на конюнктивата (84%), назална конгестия (75%) и ринорея (72%). частичен синдром на Horner, с лека ипсилатерална птоза, миоза и оток на клепача се наблюдава при 69% от пациентите. Поведението при 93% от пациентите е много типично, те не могат да продължат нормалната си активност по време на пристъпа, не стоят спокойно, не лягат в покой, не търсят помощ и не търпят присъствие на други хора.
Лечението на пристъп се провежда с инхалация на 100% кислород, кортикостероиди, триптани и интраназално lidocaine или cocaine. Профилактичното лечение се провежда по време на периода с калциеви антагонисти, кортикостероиди, lithium carbonate или антиконвулсанти.
