Abstract

One of the major advances in headache research in 2018 is the publication of the third edition of the International Classification of Headache Disorders (ICHD-3).
As recommended by the International Headache Society classification committee, clinicians and researchers should start using the ICHD-3 criteria immediately. ICHD-3 is structured in a hierarchical way and this assumes that there could be several diagnostic levels ranging from the rough to more detailed.
Since a patient may suffer from more than one headache type, each distinct type, subtype or subform of headache must be separately diagnosed and coded.
The ICHD-3 contains a substantial amount of new information, but several updates in particular merit attention: new diagnostic criteria have been included for migraine with aura that better distinguish it from transient ischemic attacks; chronic migraine, which was in the appendix of the previous versions, has been included in the main body; the classification of trigeminal neuralgia has changed on the basis of presence of degree of neurovascular contact- classical, secondary and idiopathic; for secondary headaches, new classification allows diagnosis before the causative disorder has been treated; a new entities- headache attributed to reversible cerebral vasoconstriction syndrome and headache attributed to aeroplane travel have been included; a close temporal relation between headache and overuse is no longer required for medication overuse headache.
The next edition of the International Classification of Headache Disorder is not expected until 10 years from now.
