Abstract

Psychogenic parkinsonism occurs in 10% of patients with psychogenic movement disorders. Differentiating psychogenic parkinsonism fromneurodegenerative Parkinson's disease with early psychogenic feature is a diagnostic challenge.
We present a 38 years old female with tremor of the left hand. The neurologic examination showed bradykinesia, rigidity and brisk deep tendon reflexes. Neuropsychological examination revealed a conversion disorder. The performed laboratory tests, brain CT and MRI showed no abnormalities and excluded Wilson’s disease. DAT-SPECT revealed reduced uptake in both basal ganglia, more right. The patient had excellence response to L -dopa therapy. The final diagnosis was early - onset Parkinson’sdisease with conversion disorder.
We demonstrate that DAT-SPECT is a useful diagnostic tool that can serve to distinguish pure psychogenic parkinsonism from neurodegenerative Parkinson’s disese.
